What do the #/#s mean? At 1st I thought bosses downed out of bosses total, but most of them don't have the correct # of bosses listed for the instance.

The #/# are the number of completed Achievements out of attainable Achievements.
For example, OS has 4 attainable Achievements: The Twilight Zone, Twilight Duo, Twilight Assist, and Besting the Black Dragonflight. Just like ICC and Naxx have different Halls and Quarters Achievements.
